Unfortunately, we do not participate in workers’ compensation insurance or no-fault insurance plans. If you have an open worker’s compensation insurance case or no-fault insurance case, your medical care cannot be covered under your regular insurance policy until the case has been closed (pending verification).
Please contact us if you have any questions regarding our participation with your insurance provider. If we do not take your primary insurance, your secondary insurance will not be valid.
If your insurance requires a referral, please confirm that we have it on file before your first appointment.
Although, we may participate in your insurance plan, many advanced services that we offer are not covered by insurance companies. It is imperative that you understand the limits of your insurance coverage. If you have questions about your plan you should contact your insurance company or plan administrator. We can also accept payment from your out-of-network benefits, health savings account or flexible spending account should they apply to your treatment. If we do not take your insurance, we will provide you with a detailed receipt of services performed for you to submit to you insurance company approximately one week after your visit. We expect payment by cash, check or credit card at the time that our services are provided.
